and Conditions of Enrollment
Fill out the attached form. Include a check or money order
for the full amount of the workshop made out to Alchemy and mail to Alchemy
Studio, 4022 E. Cambridge Ave., Phoenix, AZ 85008. This will reserve your
place. You will be sent a confirmation letter with further details approximately
ten days before the workshop. Including an email address on the form below
will allow for the confirmation letter to be sent via email. Materials
fees are due the day of the workshop. All classes have limited enrollment
and participants will be taken on a first come first served basis. In
the event of insufficient enrollment students will be notified prior to
the workshop and can either receive credit for another workshop or receive
a full refund. Refunds will be issued only if the workshop is cancelled.
If registering for more than one workshop, please fill out another form
for each additional class.
